AHSFHC’s mission
is to provide culturally competent and linguistically appropriate
quality and compassionate health care services to the Asian American and
all other underserved or underprivileged communities in metropolitan
Chicago
The AHS Family Health Center provides quality primary care services to
immigrants and refugees, and other medically underserved residents on
Chicago's North Side. Our staff members speak many languages and through
Asian Human Services has access to over 25 other languages and dialects.

Primary Care Services
Comprehensive
services for individuals in all stages of life are available at the
health center. Maternal health care focuses on early and regular access
to prenatal care, prenatal classes and referrals to WIC and oral health
services. Emphasis is placed on health promotion, nutrition, risk
avoidance and preparation for childbirth to increase the likelihood of
healthy behaviors and good outcomes. Pediatric services focus on
preventing childhood illnesses via immunization compliance, lead
screening, and follow-up, with complementary services focusing on child
nutrition and child safety issues. Adolescent care includes keeping
immunizations up to date and screening for risky behavior and mental
health needs, with appropriate education on healthy lifestyles, injury
prevention, mental health services, and nutrition. Services for
adult men and women focuses on disease prevention and screening –
particularly pap smears and mammograms for women, PSA screening for men,
and diabetes, hypertension, and TB screening for both genders. Services
for senior citizens focus on cancer screenings, following prescribed
immunization protocols, dental and mental health needs.
Mental Health Services
AHS Family
Health Center offers culturally appropriate mental health services for
seniors, adults and children by licensed competent professionals.
Services include individual counseling, case management, crisis
management including domestic violence related counseling and
supportive. In house psychiatric consultation is available from December
2007.
Women, Infant and Children (WIC) Program
A registered
dietician is available on site to enroll patients in the WIC
program. WIC provides nutrition education, nutrition counseling,
breastfeeding support and nutritious foods. Integrating nutrition
education, nutritious foods and yearly health care visits strengthen
families long after their WIC eligibility has ended.
All Kids – Family Care Enrollments
AHSFHC staff
assists uninsured patients to enroll in the All Kids/Family Care
program. All Kids covers doctor visits, hospital stays, prescription
drugs, vision care, dental care and eyeglasses. All Kids covers regular
check-ups and immunizations (shots). For eligibility criteria call the
above phone number and ask for the All Kids Application Agent.
Discounted Prescription Drug Program
AHS Family
Health Center partners with the local Walgreen’s to provide
prescription drugs at a discounted rate to the health center's uninsured
patients.
Opening Doors Women’s Group
AHS Family
Health Center mental health department offers stress reduction groups
on the first Tuesday of every month. Providing women a safe space to share
their experiences and this group is a great opportunity to voice feelings and learn
alternative ways of coping with stress. The primary aim of the group is
to enable women to come together, empower themselves and enhance their
general well being.
